Book an Appointment

Instructions for scheduling:

  1. Click on the link above that says "Schedule here"
  2. Choose your "appointment type" from the drop-down menu.
    1. Select "New Patient" if you have never been to our clinic before and plan to become a monthly member. Select "Non-Member Visit" if you do NOT plan on becoming a monthly member (no telehealth or online communication available for non-members). Membership is $95/month for unlimited office visits and $145/month for office + home visits. Non-member visits are priced per item. If you schedule a non-member visit and decide after that you would like to become a member, the cost of the visit will be applied to monthly membership if you sign up the same day. Call us directly to schedule home visits.
  3. You should see a calendar at the bottom of the page. If no calendar appears, hit the "More" button to view available days. If you are a member and need an appointment, but do not see a calendar date for scheduling, please contact us by phone/text or through the Patient Passport.
  4. Select your time and follow the prompts to continue as a guest or login to the patient passport if you are already a member.
  5. You will be prompted to enter payment after signing in. You will receive a confirmation email after payment is submitted. New patients will receive a copy of the patient agreement form, which needs to be reviewed and signed by you prior to your appointment. 

We are a new practice and are working on creating the best schedule for our patients. If you need to be seen at a time that is not listed on the current calendar, please reach out to us directly at 360-564-1595 and we will do our best to accommodate you.
